Use this form to submit a clear notice cancelling your distance contract. You can review your details and separately confirm cancellation before submitting it. You will then receive a reference number and email confirmation.
Using this form is optional. You may also cancel in any other clear way. Do not return the product until the seller has provided the correct return address.
